The Last Resort Animal Rescue and Sanctuary

West Milford NJ

Foster/Adoption Application

PLEASE READ BEFORE FILLING OUT THE FORM*

Please be aware that we are an ALL volunteer rescue organization and due to an incredibly high volume of emails, voicemails and applications, we are currently delayed in responding to everyone as quickly as we would like. Applications are reviewed in the order they are received . It can take an average of 5-7 days for us to review your application so please be patient and we WILL get back to you as soon as possible .   Please be sure to provide as much information as possible on your application to help us to determine the best possible match for you and your family.

Our main goal is the successful and permanent placement of each and every dog we rescue into loving forever homes.  We often must make difficult decisions  in determining which applicant/family will be the best match for each dog.  Please realize that once your application is approved, we will make every effort to find the exact right dog for each of you and we have a 99% success rate with placement. Please allow us to help you in choosing a dog that's age, size, temperment and activity level match your lifestyle to ensure a positive experience for everyone, 2-legged and 4-legged alike!

The information requested in this form will assist us in matching you to the right pet for your family, needs and lifestyle. Please understand that we are totally responsible for the all animals we rescue as they have no one else to guarantee their lives and well-being.

We take this responsibility seriously and will not adopt out to any home when we cannot feel totally confident of the adopter’s willingness and ability (financial and personal) to commit to this animals future well-being and happiness.

*ALL ANIMALS WILL BE UTD ON VACCINES, SPAYED/NEUTERED & HEARTWORM TESTED, PRIOR TO ADOPTION*

A $300 adoption fee* is required at the time of taking your new family member home.  The check can be post dated up to 2 weeks. Then, once we receive the signed contract after your trial period we will cash your check . *Occasionally in certain cases, the adoption fee may be higher based on incurred medical expenses

*All fees are used ONLY to cover the vetting and transport fees for each pet. We are a strictly volunteer rescue and we make NO profit. The adoptions fees barely cover the rescue cost for each dog and all extra donations are greatly appreciated and will go directly to saving another animal in need of rescue.

The fee is refundable for any reason within the first two weeks of your ownership provided it does not work out and the animal is returned to TLR safe and sound.

Please realize that you are not at liberty to dispose of this animal in any way, except to return her/him to us. The animal may not be sold, given away, put in a shelter or euthanized without the knowledge and permission of TLR.

This application is the first step in a process that we anticipate will culminate in the placement of a healthy and temperamentally-sound animal into your home as a new family member.

Step two will be a veterinary check. Your veterinarian will be telephoned and asked about your previous record of treatment and care of your pets.

Step 3 will be a home check, where a member or representative of TLR will visit your home to ascertain the suitability of the living quarters for your new pet and to suggest any safety features that you may not be aware of in providing a safe and secure home for your newest family member.

ONCE APPROVED, WE WILL MAKE EVERY EFFORT TO MATCH YOU WITH THE PERFECT DOG FOR YOUR FAMILY. THERE WILL BE A REQUIRED TRIAL PERIOD PRIOR TO ADOPTION WHERE THE DOG WILL LIVE WITH YOU AND YOUR FAMILY ON A DAY TO DAY BASIS. AFTER WHICH TIME, WE WILL EVALUATE YOU AND YOUR FAMILYS COMPATABILITY WITH THE DOG. PROVIDING IT IS A GOOD MATCH FOR ALL INVOLVED, THE ADOPTION WILL THEN BE FINALIZED BY A MEMBER OF TLR.

ALL Members of the household must be present for the home visit. No exceptions!!

AGAIN, WE MAKE A COMMITMENT TO EACH AND EVERY DOG WE RESCUE FOR THE REST OF ITS LIFE.

THESE ANIMALS HAVE NO ONE ELSE TO SPEAK FOR THEM OR TAKE RESPONSIBILITY FOR THEIR WELL BEING. WE TAKE THIS RESPONSIBILITY VERY SERIOUSLY!!!!!!!

THIS PROCEDURE IS ONLY FOR THEIR PROTECTION AND IS NOT INTENDED TO BE INTRUSIVE.

Please be aware that our ONLY objective is to permanently re-home these animals in the best possible environment for there specific needs and to help you to choose the pet that best suits your home and lifestyle.

We reserve the right to refuse adoption to anyone, for any reason, at any time, in accordance with our experience and sense of responsibility for the animals in our care.

Please answer every question accurately and provide first and last names and current contact info for all references or your application WILL NOT be processed.

Are you able to afford the adoption fee of $300.00 for adult dogs or $350 for puppies? Yes
No

Recently someone asked, "Why would you charge $300 for a rescue pet? Why not just place it in a good home?" The answer is because we need adopters who are willing to support all rescue pets, not just the dog they adopt. A young, healthy dog might cost us $150-400 in vet care, and many people will want to adopt it.

An older one with health issues can easily cost $500-1500 in veterinary expenses, and it may require months of foster care for rehabilitation; yet we can only ask a minimal adoption fee, and it may be a long time until anyone comes forward to adopt it.

Fees from the younger, more adoptable dogs help offset the cost of caring for the others. Most of our dogs have a sad story to tell, and some have suffered through years of neglect or abuse. We hope that you too are willing to give an opportunity for a better life to ALL of our dogs by supporting them through adoption fees. Without these funds, our rescue efforts would not be possible."
